Use the Clinic Pharmacy Mail Service Prescription Transfer process when one or more prescriptions are held by an outside pharmacy and need to be moved to the mail-service pharmacy.

Transfer preparation

Identify both the prescription and the sending pharmacy

A transfer request needs enough information for the receiving pharmacy to locate the active record. Prepare the prescription number, medication name, current pharmacy name and telephone number, prescriber details, and any refill information shown on the label.

Do not assume the prescription has moved until the receiving pharmacy confirms the transfer.

Plan around the transfer processing period

Transfers require communication between pharmacies and may take longer when information is incomplete, the prescription is expired, or no authorized refills remain. Submit the request before the current supply becomes urgent.

If the prescription cannot be transferred, the pharmacy may direct the patient to the prescriber for a new prescription.
