An originating site is where a Medicare beneficiary receives a telehealth service. Current federal flexibilities affect which locations qualify, but the answer remains tied to the service date and type of care.
What CMS says for the current period
CMS’s February 26, 2026 Telehealth FAQ states that through December 31, 2027, Medicare beneficiaries can receive telehealth services anywhere in the United States and its territories. It says that beginning January 1, 2028, most non-behavioral services will generally again require a medical facility in a rural area.
The same FAQ treats behavioral-health services differently. A location rule should therefore name the service category and date rather than saying simply that “home is always allowed” or that “rural rules always apply.”
Originating site and distant site are different
The originating site describes the beneficiary’s location. The distant site describes where the furnishing practitioner is located. Either term can affect a claim, but neither alone proves that the service, practitioner or communication technology meets every Medicare condition.
Write “for dates of service through…” and cite the current CMS FAQ. A future expiration or exception belongs in the same paragraph so readers can see the limit.

- Current Medicare telehealth service-list status.
- Eligible practitioner, technology and billing conditions.
